Palliative Care Nurse
Montrose, Full‑time Mon‑Fri 8am‑5pm with rotating on‑call/weekend. At Hope West, you will be the nurse you always dreamed of being – a source of compassion, strength, and courage for patients and families. You will guide patients in identifying their personal goals of care and support caregivers in caring for their loved ones. As a hospice nurse you are a generalist across all diseases and a specialist in symptom management, requiring excellence in assessment and critical thinking.
Primary Registered Nurses have an assigned caseload and lead the interdisciplinary team caring for those patients. Care is delivered wherever patients call home – private homes, nursing homes, group homes, or assisted living facilities.
