Responsibilities
  • Assess patient needs and develop individualized care plans with clearly defined goals.
  • Coordinate with an interdisciplinary team to implement and adjust plans of care.
  • Monitor patient status, manage the progression of the caseload, and address barriers to goals.
  • Educate patients and families about care plans, medications, symptom management, and follow-up steps.
  • Document clinical assessments, plans, and outcomes in the electronic health record; communicate with referring providers and care partners.
  • Facilitate referrals, referrals to ancillary services, and appropriate post-visit resources to support safe transitions.
Requirements
  • Active Registered Nurse (RN) license (state‑specific or as required for practice).
  • Relevant case management and clinical experience in an outpatient setting.
  • Strong assessment, critical thinking, and communication skills.
  • Ability to collaborate with an interdisciplinary team and coordinate care across services.
  • Current CPR/BLS certification or ability to obtain.
  • Education:

    RN degree (Associate Degree in Nursing or higher) as the foundational credential.
