$25–$32/hour (DOE) + Mileage Reimbursement

Summit West Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled care to patients throughout Mesa County.

This position offers flexible scheduling and the opportunity to build a caseload over time. You'll work independently in patient homes while receiving support from an experienced team of clinicians dedicated to delivering high-quality, patient-centered care designed to improve quality of life.

• Provide skilled nursing services in patient homes
• Perform wound care, medication support, and patient monitoring
• Observe and report changes or concerns promptly
• Document visits accurately and timely in the EMR
• Collaborate with RNs, therapists, caregivers, families, and providers
• Support patient education and individualized care plan goals

• Current Colorado Licensed Practical Nurse (LPN) License
• Valid driver’s license, reliable vehicle, and auto insurance
• IV Certification preferred; not required
• Strong communication and accurate documentation skills
• Ability to work independently and adapt professionally in home health environments
