Diabetes-Focused Nurse Practitioner

Job Summary:

Licensed independent practitioners who provide primary and/or specialty nursing and medical care in ambulatory and acute care settings. A Diabetes-Focused Nurse Practitioner will help manage and treat patients with Type 1, Type 2, and gestational diabetes. They prescribe medications, manage insulin pumps, and continuous glucose monitors, lifestyle and dietary counseling to prevent chronic complications. They will also complete annual wellness visits for our Medicare patients.

Duties/Responsibilities:

Patient Care Meets standards of practice as outlined by AANP.Provide direct patient care, evaluate outcomes, consult with other providers/specialists as needed and adjust nursing care processes to ensure optimal patient care. May be responsible for coordinating health education information.

Identifies patient/client needs, develops quality programs, and ensures deliver of quality services to meet those needs for all patients.

Prescribes or recommends prescriptions for medication and other treatments according to established federal and state requirements. (See position statement from AANP.)Performs, orders, and evaluates results of lab, x-ray, and diagnostic tests as indicated.

Provides coverage for other providers’ patients when a provider is unavailable. Assists in acute care, follow ups, messages and prescription refills.

Maintains professional affiliations and enhances professional growth and development to keep abreast of the latest trends in family  a contributing member of the team in assisting in the efficiency of the workflow and in the education of the Nurse Practitioner and BSN students.

Participates in problem solving teams or provides suggestions to management to improve overall quantity/quality of care and day-to-day operations.

Assists with provider coverage when applicable.

Maintains accurate and legible medical records.

Assure positive communications with the nurses, physicians, patients and families

Evaluates and monitors the effectiveness and outcome of medical nutrition therapy and diabetes education interventions.

Ability to set priorities, delegate tasks, meet multiple deadlines, analyze departmental problems, and develop and implement effective solutions.

Actively participates in nutrition task force meetings and ensures high quality delivery of care by continuous monitoring and improvement in identified areas.

Perform comprehensive physical exams, analyze medical histories, and interpret advanced lab data.

Develop and implement individualized care plans, including pharmaceutical interventions and insulin titration.

Instruct patients on carbohydrate counting, medication administration, and home glucose monitoring.

General Requirements:

Knowledge of complex areas of clinical nutrition include management of ill and malnourished patients, and ability to apply medical nutrition therapy to patients with multiple co-morbidities.

Demonstrated ability to utilize electronic health records and proficiency in other computer programs such as Microsoft Word, Excel, and Power Point Demonstrated  ability to utilize nutrient analysis software.

Demonstrates highly effective interpersonal, written, and verbal communications.

Experience:

Must be a Nurse Practitioner who can work in any medical environment; caring personality with a positive attitude; calm under pressure and exhibit sound judgment; excellent time management and problem-solving abilities; good communication skills. Prior clinical experience with diabetes management. Certification as a CDCES or Advanced Diabetes Management (ADM) is highly preferred.

Education Requirements:

Master’s Degree in Nursing. Nurse Practitioner certification licensed in the state of Colorado, advanced practice license (ANA Board) and DEA licensure. Attend Continuing Education programs as requested or as required by the position.

Working Environment:

Works in a well-lighted and ventilated area; primarily standing and walking; light to moderate lifting all the time; heavy lifting occasionally
