• 2+ years of experience
• Current and unrestricted APN and RXN license in Colorado required.
• National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or other appropriate population focus preferred
• DEA registration
• BLS/CPR certification
• Valid driver's license, acceptable driving record, and proof of automobile maintained throughout employment.

• Provides a wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, and urgent medical needs all in collaboration with the Chief Medical Director.
• Conducts health assessments and evaluates patient needs in the clinic
• Actively collaborates with the entire Medical interdisciplinary team for the highest quality care of each participant.
• Make appropriate and timely referrals to our contracted providers out in the community
