A-Line Staffing is hiring an RN Case Manager for an onsite contract-to-hire opportunity in Grand Junction, CO. This role focuses on assessing high-risk members, coordinating care across providers, and ensuring safe, cost-effective care transitions.
Schedule / Details- Schedule:
Monday-Friday, 8:00 AM-5:00 PM - Hours:
40 hours/week (40 guaranteed) - On-call:
Potentially 1 day/week and 1 weekend/month (not guaranteed) - Work setting:
Onsite - Counties covered:
Mesa County - Mileage reimbursement: $0.43/mile
- Perform comprehensive assessments of high-risk members
- Partner with primary care providers to build and execute individualized care plans
- Continuously reassess and direct members to appropriate, cost-efficient levels of care
- Arrange and coordinate care transitions with physicians, social workers, discharge planners, and other stakeholders
- Engage specialty resources as needed to resolve complex situations
- Track and document clinical, functional, and fiscal outcomes
- Identify health promotion and prevention opportunities
- Advocate for patients while protecting privacy and confidentiality
