RN Case Manager – Case Management

Schedule:

Full-Time;
Weekend Coverage: 5 hours for one Saturday each month; take 5 hours off on one day of the week.

Requirements:

• Bachelor's Degree in Nursing (Master's Degree preferred)
• Current unrestricted Registered Nurse license in the state
• At least three (3) years of clinical experience in a hospital setting
• At least five (5) years of case management experience in a hospital setting
• Certification in case management (preferred) or willingness to obtain within two (2) years of employment

Roles & Responsibilities:

• Assess, plan, implement, coordinate, monitor, and evaluate options and services to meet clients' health and human service needs.
• Maintain and enhance the case management system through ongoing interaction with patients, physicians, and healthcare providers.
• Document case management plans clearly and concisely.
• Collaborate with interdisciplinary teams to facilitate effective and cost-efficient patient care.
• Educate staff and physicians on appropriate levels of care and utilization issues.
• Stay current with case management practices through ongoing education and participation in professional activities.
