Registered Nurse (RN) - Home Health

We are seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position, offering flexibility in how you structure your day. The position includes manageable caseloads and meaningful one-on-one care.

What You'll Do:

• Perform comprehensive patient assessments.
• Develop and update individualized plans of care.
• Provide skilled nursing care and appropriate interventions.
• Monitor patient progress and report changes to the care team.
• Educate patients and families on medications, treatments, and care plans.
• Coordinate care with LPNs, therapists, caregivers, and providers.
• Complete timely and accurate documentation in EMR.
• Participate in on-call rotation.

Work Environment:

Home health offers a different pace and structure than facility-based care. Our clinicians enjoy independent visits in patient homes.
