Licensed Practical Nurse

Mantey Heights Rehabilitation & Care Center, a Stellar Living community in Grand Junction, CO, is seeking a compassionate LPN to join our Healthcare & Medical Services team. In this role, you will provide high-quality clinical care, administer medications, monitor resident health, and collaborate with an interdisciplinary team to support individualized care plans. Responsibilities

• Provide direct nursing care to residents in assisted living, memory care, and rehabilitation settings
• Administer medications and treatments according to physician orders and facility protocols
• Monitor and document vital signs, symptoms, and changes in resident condition
• Update and follow individualized care plans in collaboration with RNs and providers
• Accurately document all care and observations in the electronic medical record (EMR)
• Assist residents with activities of daily living as needed, promoting dignity and independence
• Support residents with mobility, safety, and rehabilitation goals
• Respond promptly to call lights and urgent resident needs
• Maintain infection control standards and a clean, safe environment
• Communicate effectively with residents, families, and interdisciplinary team members
• Educate residents and families about health conditions, medications, and care plans
• Participate in care conferences and shift handoffs, providing thorough reports
• Supervise and mentor caregivers, ensuring quality and consistency of care
• Follow all state, federal, and facility regulations, policies, and procedures
• Work flexible shifts, including weekends and holidays, as part of a 24/7 care team
