Mantey Heights Rehabilitation & Care Center, a Stellar Living community in Grand Junction, CO, is seeking a compassionate LPN to join our Healthcare & Medical Services team. In this role, you will provide high-quality clinical care, administer medications, monitor resident health, and collaborate with an interdisciplinary team to support individualized care plans. You'll build meaningful relationships with residents and families while promoting wellness, dignity, and engagement.

Responsibilities
- Provide direct nursing care to residents in assisted living, memory care, and rehabilitation settings
- Administer medications and treatments according to physician orders and facility protocols
- Monitor and document vital signs, symptoms, and changes in resident condition
- Update and follow individualized care plans in collaboration with RNs and providers
- Accurately document all care and observations in the electronic medical record (EMR)
- Assist residents with activities of daily living as needed, promoting dignity and independence
- Support residents with mobility, safety, and rehabilitation goals
- Respond promptly to call lights and urgent resident needs
- Maintain infection control standards and a clean, safe environment
- Communicate effectively with residents, families, and interdisciplinary team members
- Educate residents and families about health conditions, medications, and care plans
- Participate in care conferences and shift handoffs, providing thorough reports
- Supervise and mentor caregivers, ensuring quality and consistency of care
- Follow all state, federal, and facility regulations, policies, and procedures
- Work flexible shifts, including weekends and holidays, as part of a 24/7 care team
