Position: REGISTERED NURSE

Mantey Heights Rehabilitation & Care Center seeks a compassionate Registered Nurse to provide skilled, resident-centered care in our Healthcare & Medical Services department. In this role, you will perform assessments, administer medications and treatments, coordinate individualized care plans, and support rehabilitation for seniors in a skilled nursing and long-term care setting. You'll collaborate closely with an interdisciplinary team to promote comfort, dignity, and independence, while mentoring CNAs and supporting families.

Responsibilities

  • Provide comprehensive nursing care to residents in skilled nursing and long-term care settings
  • Conduct ongoing assessments and document resident conditions and responses to treatment
  • Administer medications, treatments, and therapies per provider orders and facility protocols
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • Support rehabilitation goals by coordinating with therapy staff and encouraging resident participation
  • Supervise, mentor, and support CNAs and other care staff to ensure high-quality resident care
  • Educate residents and families on care plans, health conditions, and discharge/transition needs
  • Maintain accurate, timely clinical documentation in the electronic health record
  • Promote resident comfort, dignity, safety, and independence in all interactions
  • Adhere to infection control, regulatory, and quality standards; participate in quality improvement initiatives
