LPN, Nursing Home, LPN​/LVN

La Villa Grande Care Center, a Stellar Living community in Grand Junction, CO, is seeking an LPN to provide compassionate nursing care to seniors in our skilled nursing and rehabilitation setting. In this resident-centered role, you will administer medications and treatments, monitor resident conditions, support care planning, and collaborate with an interdisciplinary team. You'll help promote safety, dignity, and quality of life while mentoring support staff and benefiting from ongoing training, professional growth, and a supportive, team-focused culture.

Responsibilities

• Provide direct nursing care to residents following care plans and physician orders
• Administer medications and treatments accurately and timely
• Monitor, document, and report changes in resident condition to RN and providers
• Support rehabilitation and long-term care goals in collaboration with interdisciplinary team
• Assist with admissions, discharges, and care conferences
• Educate residents and families on care, treatments, and health management
• Ensure compliance with infection control, safety, and regulatory standards
• Supervise and support CNAs and other support staff during assigned shifts
• Maintain accurate electronic health records and required documentation
• Promote a resident-centered, dignified, and compassionate care environment
