LPN, LPN​/LVN, Nursing Home

Mantey Heights Rehabilitation & Care Center in Grand Junction, CO seeks a compassionate LPN to provide direct nursing care to skilled nursing and long-term care residents. Responsibilities include medication administration, treatments, wound care, and accurate EHR documentation while monitoring residents' conditions and reporting changes. The LPN will support care plan development, coordinate with RNs, CNAs, therapy, and providers, and educate residents and families.

Responsibilities

• Provide direct nursing care to skilled nursing and long-term care residents within LPN scope of practice
• Administer medications and treatments safely and accurately
• Perform wound care, vital signs monitoring, and basic patient assessments
• Document care, observations, and interventions in the electronic health record
• Monitor residents' conditions and promptly report changes to the RN or provider
• Support development and implementation of individualized care plans
• Collaborate with RNs, CNAs, therapy staff, and providers to coordinate resident care
• Educate residents and families on care plans, treatments, and health management
• Follow infection control, safety, and regulatory standards at all times
• Contribute to a resident-centered, compassionate, and team-oriented culture
