CNA, Nursing Home

Mantey Heights Rehabilitation & Care Center in Grand Junction, CO is seeking a compassionate CNA to provide hands-on care for skilled nursing, rehab, and long-term care residents. The CNA will assist with ADLs, mobility, toileting, feeding, and hygiene while monitoring vital signs and promptly reporting changes to nursing staff. You'll support therapy and restorative plans, uphold infection control and safety standards, and maintain accurate documentation.

Responsibilities

• Provide direct resident care including bathing, dressing, toileting, feeding, and grooming
• Assist residents with safe transfers, ambulation, and use of mobility devices
• Measure and document vital signs and report changes in condition to nursing staff
• Support therapy and restorative care plans as directed by nursing and rehab teams
• Maintain accurate and timely documentation in charts or EMR systems
• Follow infection control, safety, and fall prevention protocols at all times
• Promote residents' dignity, comfort, and independence in all interactions
• Communicate effectively with residents, families, and interdisciplinary team members
• Assist with room set-up, cleanliness, and equipment organization
• Participate in team meetings and ongoing training to improve care quality
