Sumo Medical Staffing is seeking an experienced Nurse Practitioner for a PACE (Program of All-Inclusive Care for the Elderly) locum assignment in Grand Junction, Colorado. This role provides comprehensive care to elderly patients within a clinic-based and community-focused healthcare model.

Work Environment & Schedule.- Schedule:
4x10-hour shifts. - No holiday coverage required.
- Primary Care Clinic setting.
- In-home visits as needed.
- Interdisciplinary PACE care team environment.
- Primary care management.
- Preventive care.
- Urgent care services.
- Assessments across multiple care settings.
- Community referral coordination.
- Collaboration with PACE team members.
- Active, unrestricted Colorado NP license.
- Board Certified.
- Minimum 2 years of experience.
- DEA.
- ACLS.
- CV in MM/YYYY format with all work and education gaps explained.
