Summit West Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care throughout Mesa County. Home health offers a different nursing experience. Instead of rushing between rooms or managing overwhelming patient assignments, you’ll provide meaningful one-on-one care, build lasting relationships with patients and families, and see the impact of your work firsthand. • ** Active Registered Nurse (RN) License in the State of Colorado** (or active NLC Compact License)
• Valid
  ** driver's license, reliable transportation, and auto insurance**
• Strong

  ** skills
• Ability to
  ** work independently
  ** and manage a structured caseload
• ** Clear, professional communication
  ** with patients, families, and team members
• Comfort with
  ** documentation and EMR systems** *(HCHB preferred)*
• Home health experience a bonus, but not required

• Perform comprehensive patient assessments
• Develop and update individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and communicate changes to the care team
• Educate patients and families regarding medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers
• Complete timely and accurate EMR documentation
  
  Participate in on-call rotation
