Summit West Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care throughout Mesa County. Home health offers a different nursing experience. Instead of rushing between rooms or managing overwhelming patient assignments, you’ll provide meaningful one-on-one care, build lasting relationships with patients and families, and see the impact of your work firsthand. You’ll enjoy the flexibility and autonomy to structure your day while receiving support from an experienced interdisciplinary team that values communication, collaboration, and quality care.

- ** Active Registered Nurse (RN) License in the State of Colorado** (or active NLC Compact License)
- Valid
** driver's license, reliable transportation, and auto insurance** - Strong
** assessment and critical thinking
** skills - Ability to
** work independently
** and manage a structured caseload - ** Clear, professional communication
** with patients, families, and team members - Comfort with
** documentation and EMR systems** *(HCHB preferred)* - Home health experience a bonus, but not required
- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and communicate changes to the care team
- Educate patients and families regarding medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Complete timely and accurate EMR documentation
Participate in on-call rotation
