American Traveler is hiring a Primary RN for a Community Hospice position requiring 2+ years of experience and a CO or compact RN license.
Requirements- Active RN license (CO or Compact) required
- Minimum of 2 years of nursing experience required
- Community-based hospice setting providing care in patients' homes, nursing homes, group homes, or assisted living facilities
- Manages an assigned caseload of hospice patients and leads the interdisciplinary care team
- Schedule is Monday through Friday, 8am-5pm
- Occasional weekend and on-call rotation required
- Serve as a generalist across disease processes and a specialist in symptom management
- Provide patient and caregiver education to support personal goals of care and build caregiver confidence
- Lead and coordinate care with the interdisciplinary team for assigned patients
- Deliver compassionate, personalized hospice care in a supportive work environment
