Position: HOUSING CONNECTOR CASE MANAGER - TEMP | Human Services

Essential Job Functions
  • Responsible for a variety of duties to assist the Child Welfare Division.
  • Communicates effectively with multiple teams within the Child Welfare Division, community agencies, and families/young adults.
  • Provide support to youth/young adults who are eligible for the Fostering to Success and Foster Youth to Independence Housing Vouchers.
  • Assist youth/young adults with applying for the voucher.
  • Assist youth and young adults in locating housing (take them to appointments, connect with landlords, tour apartments and complete the housing checklist/inspection to ensure that the house meets Department of Local Affairs standards and is safe).
  • Assist youth/young adults with any barriers to getting housing (i.e., obtaining vital documents, paying application fees, connecting to other resources, purchasing phones).
  • Manage the vouchers and provide case management once the youth are housed.
  • Landlord recruitment and collaboration.
  • Assist in spending funding for housing needs.
  • Create resources to inform the community about our programs and housing needs.
  • Cultivate and maintain relationships with local businesses and other community organizations.
  • Oral and written communications must be clear and effective.
  • Perform work of moderate difficulty with multiple projects and interruptions. Compile information, prepare reports, track and process documents, and enter and retrieve information in a variety of automated systems, including but not limited to the State System (Trails), Google Forms, and Excel spreadsheets.
  • Adhere to strict compliance with maintaining confidentiality when reviewing records, which may contain sensitive information pertaining to child abuse and neglect, sexual abuse, severe trauma, mental health diagnosis, substance abuse diagnosis, medical history, and child fatality.
  • Perform other related duties as assigned.
Qualifications

Education and Experience:

Bachelor's degree in a Human Services related field. Any combination of education, training, and experience which provides the knowledge, skills, and abilities required for the job.

Physical Requirements and Working Environment

This position requires standing, stooping, sitting, bending, twisting, and lifting up to 10 pounds. Work is generally confined to a standard office environment.
