DescriptionThe Service Coordinator is directly responsible for the service provision and housing assistance for the residents at Grand View in the Affordable Housing Portfolio. This program serves seniors and persons with disabilities located at one of the affordable housing sites. Service modalities include Trauma Informed Care, Harm Reduction, and Motivational Interviewing. The position works in conjunction with the Property Manager at the housing site, conducting needs‑based assessments, community outreach, case management, and facilitating health, wellness, and educational programs tailored to resident needs to improve well‑being and foster a strong sense of community.
EssentialDuties And Responsibilities
- Ensures that the resident’s voice is heard, and their ideas play a prominent role in programming.
- Within program guidelines, facilitates and accepts referrals on residents from the Property Manager, family members, service providers and the residents themselves.
- Assesses services needed by residents in the building through surveys, visitations, group meetings, and completes assessments and collects needed information. Identifies and serves the unique needs of the resident and utilizes screening and assessment.
- Facilitates the delivery of appropriate services for the households as indicated by relevant service modalities through an equity lens.
- Provides the residents with information on specific services and/or program providers, giving several options whenever possible. Complies with all Fair Housing requirements.
- Encourages the resident and/or family members to arrange for required services, when possible, to maintain and encourage their independence.
- Coordinates and monitors the services and activities of providers to residents to assure quality of care and cost effectiveness. Advocates for residents per their wishes.
- Maintains an up‑to‑date resource guide on service providers in the community by becoming familiar with local resources through Area Agency on Aging and using the National Council on Aging’s Benefits Checkup.org website.
- Maintains and submits accurate and updated records on all residents to include the services they receive.
- Develops support groups and workshops in response to needs and facilitates or co‑facilitates these groups on a regular basis.
- Assists in relocating residents to a higher level of care as needed.
- Supports team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.
- Is responsible for implementing the Service Coordinator program in accordance with HUD and Volunteers of America guidelines.
- Attends orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested and applicable to meet the needs of the position. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.
- Clearly documents all client interactions along with required eligibility and demographic information.
- Performs duties in a professional manner by maintaining the confidentiality of all information and by participating effectively within and across teams.
- Must complete annual CEU requirements.
- Performs job responsibilities in accordance with the Social Work Code of Ethics.
- Performs all other duties as assigned.
$20.00‑$22.00 per hour (Based on experience)
