Overview

Position:
Registered Nurse (RN) – Bilingual English & Spanish. The role involves coordinating care, ensuring compliance with federal/state regulations, managing clinic operations, and maintaining patient records for Compass Connections’ health program.

Language Requirement

Fluency in English and Spanish is required.

Academic & Certification Requirements
  • Graduate of an accredited nursing school; a bachelor’s degree in a relevant field is preferred.
  • Current, unrestricted LVN or RN license in the state.
  • Current BLS certifications (PALS preferred); first aid and CPR.
Work Experience
  • Minimum one (1) year in public health or a healthcare setting with care coordination experience.
  • Pediatric nursing experience is preferred.
Critical Action Items & Measurable Deliverables
  • Meet and maintain all federal and state regulatory guidelines applicable to the position.
  • Generate, decipher, and correct medical documentation in accordance with federal guidelines.
  • Validate completion of all clinic duties through daily audits of portal documentation.
  • Perform duties to ensure patient safety: assess health conditions, assist procedures, operate equipment, administer vaccines, medications, monitor side effects, and provide patient teaching.
  • Validate record systems for required facility documentation and reporting to meet federal guidelines.
  • Communicate and consult with the Lead Medical Coordinator and Regional Director on policy and procedure issues.
  • Participate in development of health and medical protocols for the federal program.
  • Attend scheduled meetings, team decisions, and operations, ensuring communication of health needs, scheduling appointments, coordinating transportation, and providing interpretation services.
  • Maintain confidential health records in compliance with PHI requirements and Compass Connections confidentiality protocols.
  • Recognize signs of abuse, neglect, exploitation, and respond professionally, reporting, and documenting.
  • Plan, organize, and prioritize daily work using sound judgment.
  • Know Compass Connections safety protocols, including evacuation procedures.
  • Maintain confidentiality in all service areas.
  • Maintain professional and ethical standards, demonstrate respect, and comply with dress code.
Other Responsibilities
  • Collect, enter, and ensure documentation for medical staff at designated sites.
  • Supervise and coordinate medical staff for clinic duties.
  • Direct, train, and coordinate services following federal, state, and company policies.
  • Monitor health care plans for the service population.
  • Ensure accurate documentation per agency policies, contract and regulatory requirements.
  • Meet all program and federal partners’ documentation requirements.
  • Perform other duties as assigned.
Requirements
  • Pass a pre‑employment drug screen and random drug screens.
  • Provide proof of work eligibility status.
  • Pass a pre‑employment and biennial criminal background check.
  • Ability to:
  • Respond sensitively to cultural & socio‑economic characteristics.
  • Work collaboratively with staff, providers, and professionals.
  • Provide crisis intervention as trained.
  • Operate in a fast‑paced environment with emotional control.
  • Maintain computer literacy.

  • Knowledge of Compass Connections policies and procedures.
  • Provide current physical‑medical release and proof of immunizations (MMR, Varicella, HepB, HepA, Tetanus, PPD, Meningococcal preferred).
  • Perform physical activities: walking, lifting patients, stretching, bending.
  • Work in a team environment.
  • Maintain professional communication skills.
