• Conduct thorough patient assessments and develop individualized care plans
• Provide skilled nursing care according to physician orders and established care protocols
• Educate patients and families on health maintenance, disease prevention, and care instructions
• Maintain accurate and timely documentation
• Collaborate with the Bright Star Care team and other healthcare professionals
• Provide compassionate, patient-centered care that promotes the best possible outcomes

• Active RN license in the state of practice
• Previous home health or acute care experience preferred
• Strong clinical, organizational, and communication skills
• Compassionate and patient-centered approach to care
• Commitment to providing safe, high-quality nursing services
