Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our Lacks Enterprises - Primary Care team in Kentwood, Michigan. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.

Opportunity Details

• Position Type: Full-Time (1.0 FTE)
• Schedule: Monday to Friday, 8:30am to 5pm
• No call, weekends or holidays
• Location: 4365 52nd St SE Kentwood, MI 49512
• Specialty: Occupational Medicine, Sick/Urgent Visits for primary care and Personal Health Assessment reviews
• Additional Details:
  • Lacks Primary Care offices are operated by Trinity Health for patients and their families enrolled in the Lacks Medical Plan
  • Pharmacy and lab on site for patients

Requirements

• Provider experience required
• APPs: Board certified in specialty prior to employment
• Active Michigan medical license (or ability to obtain)
• DEA license
• ACLS/BLS certification (must be maintained)

Duties & Responsibilities

A typical day may include:

• Providing direct patient care
• Collaborating with multidisciplinary teams
• Participating in teaching and mentoring
• Documenting in Epic EMR
• Engaging in quality improvement initiatives
• Supporting community health efforts
