Position OverviewPhysical Therapist. PTs provide professional physical therapy services to the organization’s clients as prescribed by the physician, in compliance with the state’s professional practice act for physical therapy, any applicable licensure/certification requirements, and the organization’s policies and procedures.

Position Highlights- Accurately assesses clients at admission and other required time points and utilizes appropriate standardized documentation.
- Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.
- If therapy only case, informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated.
- Makes appropriate referrals and coordinates care with others to ensure effective and efficient care is provided.
- Reports changes in client condition in a timely manner to the client’s physician and obtains orders for changes in the plan of treatment.
- Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients.
- Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.

- Possession of a licensed degree from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.
- Licensed to practice as a physical therapist in the state of practice.
- Minimum of one (1) year experience or a preceptor for a minimum of 6‑8 weeks.
- Current working knowledge of conditions of participation and home healthcare experience preferred.
- Ability to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with agency clients and families.
- Current driver’s license and reliable transportation to and from work site.
