1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas. $$$ Sign on Bonus $$$
The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients
Duties Include:- Perform start of care visits and establish care plans. Monthly follow up visits.
- Perform Supervisory Visits to client and train and supervise caregivers
- Collaborate with interdisciplinary groups and coordinate care
- Handle emergency phone lines on off hours
- Participate in community marketing and customer relation activities
- On occasion, assist Clinical Manager with Quality Assurance chart audits
Salary commensurate with experiencebeginning at $75,000 annually with $5,000 sign on bonus to be paid in instalments
