Location: Care Resources PACE – Grand Rapids, MI

Schedule: Monday–Friday, 8:00 AM – 4:30 PM (no weekends!)

Employment Type: Full-Time

This is a dynamic, people-facing position where you’ll enjoy variety in your day. Work with the participants in this amazing program at our main facility in Kentwood with some home visits each week. Plus, enjoy a consistent weekday schedule with no weekend commitment for a great work-life balance!

• Perform intake screenings and make appropriate recommendations for services.
• Function as a key member of the interdisciplinary team, collaborate on care planning and service allocation decisions.
• Complete participant evaluations as needed and develop plans of care.
• Provide treatment to participants and implement physical therapy procedures consistent in achieving identified goals.

• Physical Therapist license in the state of Michigan
• Minimum of 1 year of experience working with a frail or elderly population
• Ability to multi-task, work independently as well as within a team setting
• Have a passion for those whom we serve through this amazing program
