- Per-Diem RN (RN, BSN, MSN)
- Per-Diem Hospice Aide (CNA)

In these roles, you'll:
- Provide care and support to patients and families during a variety of different schedules
- Triage and respond to after-hours patient needs via phone and in-person visits
- Collaborate with an interdisciplinary team to develop and implement plan of care
- Conduct informational visits and complete possible assessments as needed
- Deliver exceptional, holistic care that honors the end-of-life journey
- 2 shifts a month
- 2 weekend shifts per quarter (every 3 months)
- 1 holiday a year
