Grand Rapids home for veterans is seeking dedicated and skilled Licensed Practical Nurses (LPNs) to support its Long-Term Care/Assisted Living units. This role involves providing high-quality resident care, demonstrating strong clinical judgment, and coordinating with interdisciplinary teams to ensure safe and effective care delivery. Staff in this position will manage direct patient care, support daily living needs, and uphold professional nursing standards.
Key ResponsibilitiesProvide comprehensive nursing care to residents in a long-term care setting
Utilize critical thinking and sound clinical judgment in all aspects of care
Coordinate care plans and collaborate with interdisciplinary teams
Maintain accurate documentation using Point Click Care (PCC)
Uphold all federal, state, and facility-specific regulations and standards
Support the workflow, goals, and daily operations of the LTC unit
Schedules may include every other weekend, 36–40 hours per week.
Active Licensed Practical Nurse (LPN) license in the state of practice
Current BLS certification
Must meet all federal, state, and local employment requirements
Additional requirements:
TB screening, health certificate, onboarding compliancePoint Click Care (PCC) experience required
Must be able to work every other weekend
Local LPN – Long-Term Care Pay Rate: $32.50/hr Estimated Weekly Pay: TBD
Travel LPN – Long-Term Care Hourly Pay: $16/hr Meals: $833 Housing: $450 Estimated Weekly Pay: $1,859
