We are seeking a Registered Nurse Case Manager to join our compassionate team at Hospice of Michigan. In this role, you will be responsible for coordinating patient care and ensuring that our patients receive the highest quality of hospice services. Your work will directly contribute to our mission of providing comfort and support during critical moments.
How you will make an impact- Coordinate and manage patient care plans in collaboration with interdisciplinary teams.
- Conduct comprehensive assessments of patients’ needs and develop individualized care plans.
- Ensure compliance with HIPAA regulations and maintain patient confidentiality.
- Provide education and support to patients and their families regarding end‑of‑life care options.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Facilitate communication between healthcare providers, patients, and families throughout the care process.
- Document all patient interactions and updates accurately in electronic health records.
- Current Registered Nurse (RN) license in the state of Michigan.
- Experience in case management or hospice/palliative care preferred.
- Strong understanding of geriatric care principles and practices.
- Excellent communication skills with the ability to empathize with patients and families.
- Knowledge of HIPAA regulations and commitment to maintaining confidentiality.
- Ability to work independently as well as collaboratively within a team environment.
