Job Title:

RN Home Health - Annapolis, MD (Local or Travel)

Assignment Details:

13 weeks, Mon
-Fri, 8am - 5pm, 40 hours per week

Home office is located in Grand Rapids, MI but we have large coverage area. This position will cover the area from Grand Rapids to Muskegon and down to Holland. RN will have an LPN to supervise and will need to case manage the case load until a replacement is found and trained.
HH and HCHB required
No guarantee, FT available
Particiaption in on call required if requested

Qualifications:

HCHB, Oasis, and HH experience required

• Experience:
  
  2+ years in Home Health
• HCHB (Homecare Homebase) & OASIS experience:
  Required
• License:
  Active RN License in the practicing state
• CPR Certification:
  Required
• Driver's License:
  Required
• Transportation:
  Must have reliable transportation and valid insurance
• CHPN Certification:
  Preferred
• Education:
  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program ( Required )
  • Bachelor of Science in Nursing ( Preferred )

• OASIS - Required
• Strong clinical judgment and patient assessment skills
