Nurse Navigator - Indian River Family Practice

Location and Schedule

Location:

3860 S Straits Hwy, Indian River, MI

Schedule:

Monday – Friday – Day Shift – Can work 8 or 10‑hour shifts

• Act as a liaison between patients, professional staff, and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.
• Serve in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality and cost‑effective health care services.
• Assess plans, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.
• Support quality improvement initiatives and adherence to safety standards.

• Licensed RN in the State of Michigan
• Experience in complex chronic disease management preferred
• Bachelor of Science degree in nursing preferred
• Five years' (5) experience as an RN preferred
