Home Health or Hospice Registered Nurse

Join our team as a Home Health or Hospice RN. Advisa Care focuses on compassionate, personalized care for patients in the Traverse City, Kalkaska, Baldwin, Cadillac, Manistee, and Bellaire areas.

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs
• Implement individualized plan of care and revise as needed
• Ensure compliance with Advisa Care policies, Medicare regulations, and other relevant requirements
• Consult with and educate patients and families about disease processes, self‑care techniques, end‑of‑life care, and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and participate in IDG conferences
• Participate in Home Health and Hospice performance improvement programs
• Maintain high level of client, family, and caregiver satisfaction
• Communicate information using current processes and technology within the organization

• Current Michigan RN license in good standing
• Minimum 1 year of home health or hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily and weekly office meetings as indicated
• Ability to adapt to patients’ status and needs
