Position Overview

We offer a $10,000 sign‑on bonus for this RN Home Care Nurse position in Grand Rapids, Michigan.

• Work under the direction of the RN/Mid-level Provider/Physician and in collaboration with other health care team members to assess, plan, implement, and evaluate patient care.
• Provide education to patients and families about treatments, medications, equipment, and related care activities.
• Document care provided via computer/written patient record in accordance with guidelines.
• Visit and care for 5–7 patients per shift in the Grand Rapids, Kent County, Ottawa County, and surrounding areas.

• High School Diploma or equivalent.
• MI Driver’s License.
• MI Practical Nursing (LPN) license.
• Basic Life Support (BLS) certification from AHA or ARC, required within 90 days.

Hours:

8 a.m. to 4:30 p.m., Monday to Friday (every fourth weekend). Weekly scheduled hours: 40.
