Inpatient Clinician - Per Diem​/Weekends

Responsibilities

Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108‑bed psychiatric facility that serves children, adolescents and adults. It is licensed by the State of Michigan, fully accredited by The Joint Commission and brings more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems.

As a part of its continued success and growth, the hospital is seeking qualified applicants for an Inpatient Clinician
.

The Inpatient Clinician provides psychiatric case‑management and therapeutic services to assigned patients with diverse needs, ensuring coordination of care between the hospital, community, treatment, therapeutic services, education and discharge planning.

• Completes a thorough psychosocial assessment and collaborates with the patient to develop individualized treatment goals.
• Initiates discharge planning and arranges after‑care appointments/referrals.
• Oversees the treatment planning process with person‑centered planning and coordinates all disciplines in treatment interventions, updating treatment plans every seven (7) days or as needed.
• Conducts group therapy and other groups as assigned.
• Facilitates individual and/or family sessions as needed.
• Documents each interaction with patients in a manner consistent with hospital standards.
• Attends all appropriate team meetings as required.
• Completes other duties as assigned.

• Must have one of the following licensures in the state of Michigan: LMSW, LLMSW, LLPC, LPC, LLP, TLLP.
• Previous group therapy experience is preferred but not required.
