Schedule:

• Monday to Friday, 8 a.m. to 5 p.m.
• Weekend on‑call rotation

This RN Case Manager will practice the full scope of nursing while building deep, meaningful relationships with patients, families, physicians, facilities, and interdisciplinary team members. The role involves coordinating comprehensive care across settings, assessing and responding to patients’ and caregivers’ needs, and ensuring quality documentation.

• Graduate of an accredited school of nursing; ASN or BSN required
• License as a Registered Nurse in the State of Michigan
• Minimum one year of clinical nursing experience
• Valid driver’s license, reliable transportation, and auto insurance
• Hospice or home health experience preferred

• Proficient nursing skills per accepted standards
• Ability to assess and respond to diverse patient and family needs in home or facility settings
• Strong coping skills for emotional stress and tolerance of varied lifestyles
• Multitasking, documentation, and deadline management

$77,000 - $85,000 USD
