Exciting Management Opportunity for LPN – Full time Assisted Living!

Holland Home in Grand Rapids is thrilled to announce a rare opening for an LPN to join our Assisted Living Management Team at our beautiful Raybrook Campus! In this role, you will manage a licensed Home for the Aged unit with 40-50 residents, overseeing daily operations and leading your team to ensure the highest quality of resident care. This full-time position operates Monday through Friday during the day, with participation in the Manager On-Call rotation alongside five other Unit Managers.

• Provide daily oversight of resident care, supervising and supporting care aides to ensure high-quality care.
• Manage residents' admissions, transfers, and discharges, ensuring a seamless transition process.
• Facilitates communication among residents, families, physicians, and departments to ensure quality care, and address concerns.
• Serve as the backup trainer for medication administration, guiding staff on proper protocols.
• Process physician orders accurately and efficiently to ensure timely implementation of care plans.
• Occasionally conduct wound care evaluations to assess and manage residents' needs.
• Complete comprehensive assessments to evaluate residents' health and well-being regularly.

• Valid LPN license in the state of Michigan
• 2 years of geriatric nursing leadership experience
• Strong care management experience and a proven track record in employee relations
• Excellent communication skills, with a focus on effective follow-up and support for both residents and staff
