LPN Home Care Jenison Grandville

About the Position

Corewell Health is offering a sign‑on bonus of up to $10,000 for nurses serving the Jenison and Grandville areas of Home Care. The service area includes West‑Central Michigan, covering locations such as Ludington, Baldwin, Nunica, Kent County, Ottawa County, and as far south as Gun Lake. Nurses will visit between 5–7 patients per shift, providing skilled nursing care in their homes.

Under the direction of the RN/Mid‑level Provider/Physician and in collaboration with other members of the health care team, you will:

• Contribute to the assessment, planning, implementation, and evaluation of patient care.
• Inform patients and families about treatments, medications, equipment, and related activities.
• Document care provided via computer/written patient records in accordance with guidelines.
