What You'll DoAs our Home Health Registered Nurse Clinical Manager
, you'll be the heartbeat of our clinical operations. Your days will be filled with purpose as you:
- Support and mentor a dedicated team of nurses and caregivers
- Ensure our patients receive safe, high-quality, personalized care
- Oversee clinical documentation, care plans, and regulatory compliance
- Collaborate with physicians, referral partners, and families
- Lead case conferences and guide clinical decision-making
- Help onboard, train, and inspire new team members
- Keep a close eye on quality measures, patient satisfaction, and outcomes
This RN role blends leadership, clinical expertise, and relationship-building — perfect for someone who loves both people and process.
Requirements What You Bring- Active MI RN license
- 2-3 years of clinical experience in home health, hospice, or a related field is preferred
- Previous leadership or supervisory experience
- Solid understanding of Medicare home health OASIS is a plus
- Comfort with EMR systems and clinical workflows
- Experience working with physicians, referral partners, patient families, and interdisciplinary care teams
- Bachelor of Science in Nursing (BSN) is a plus
- Experience with Kinnser is a plus
- Ability to work in the field as needed
