PRN RN Field Nurse
The PRN RN Field Nurse, under the direction of the Supervising Nurse, is responsible for delivering comprehensive patient care through assessment, planning, and direct nursing services. The Staff Nurse conducts initial patient evaluations, performs Start of Care (SOC) visits, and completes monthly supervisory visits to update patient charts and document new details.
Essential Duties- Provide guidance to patients and families/guardians by overseeing medical care to promote independence, wellness, and illness prevention through education and self-empowerment.
- Collaborate with patients, families/guardians, and the rehabilitation/care team (therapists, aides, physicians, etc.) to ensure continuity of care and facilitate necessary home modifications.
- Monitor and evaluate all services to ensure infection control, safety precautions, and medical requirements are met, while supporting patients in achieving expected goals.
- Develop, implement, and update comprehensive, goal-focused care plans with preventive and rehabilitative nursing procedures.
- Conduct planned home visits for assessment, instruction, supervision, and nursing treatments according to the care plan. Provide guidance to aides delivering services, including ongoing supervision and evaluation.
- Share responsibility for on-call duties in emergency or staffing situations, which may involve providing off-hours nursing visits or shift coverage.
- Maintain continuous communication with the patient's physician to report progress, changes in status, or the need for additional services.
- Interpret agency services and policies to patients, families, physicians, and community partners, while ensuring compliance with patient care rights and responsibilities.
- Organize daily visit schedules to maximize productivity and minimize travel time.
- Inform the Supervising Nurse of services provided, patient needs, and any changes to the plan of care or schedule.
- Perform other duties as assigned.
