Role:
Hospice Registered Nurse Case Manager
Responsibilities:Provide compassionate, holistic care to patients in their homes, develop and implement individualized care plans, collaborate with interdisciplinary teams, educate and support families and caregivers, and serve as a calming, steady presence during challenging times.
Qualifications & Skills- Current RN license in Michigan
- Experience in hospice, home care, acute care, or case management
- Ability to work independently with a team-oriented mindset
- Strong emotional resilience and compassion
Support patients and families with clinical and emotional care, coordinate with healthcare providers, educate on care processes, and be available for on-call needs as required.
