Role Overview
An Admissions RN provides compassionate care to patients and families beginning their hospice journey. Responsibilities include conducting initial assessments, education, and admissions in various settings, explaining hospice services, and coordinating with interdisciplinary teams for seamless transitions. The role involves emotional support, documentation, and ensuring eligibility criteria are met.
Qualifications & SkillsCandidates must hold a current RN license in Michigan, with at least 2 years of clinical experience, preferably in hospice, palliative, or acute care. Excellent communication, organizational, and documentation skills are essential, along with a valid driver's license and reliable transportation for local travel.

- Complete documentation and coordinate with team members
