Emmanuel Hospice is seeking an experienced and compassionate Clinical Team Coordinator to provide leadership and operational oversight for interdisciplinary hospice care teams. This role is responsible for ensuring exceptional patient care, efficient clinical operations, regulatory compliance, staff development, and collaboration across disciplines. The Clinical Team Coordinator plays a critical role in advancing our mission of delivering compassionate, patient-centered end-of-life care

Qualifications:
- Valid Michigan licensure as a Registered Nurse (RN),BSNpreferred
- Minimum of 5 years of healthcare experience with hospice experience preferred
- Previous leadership,manage mentor supervisory experience preferred
- Proficiency with Electronic Medical Records (EMR) systems and Microsoft Office applications.
Responsibilities include:
- Oversee daily clinical services to ensuretimelypatient care and appropriate utilization of clinical resources.
- Monitor staffing coverage, visit completion, and workflow challenges, addressing operational issues promptly and effectively.
- Partner with Scheduling tomaintainadequate staffing andoptimizeclinical productivity.
- Lead daily team prioritization and staff performance efforts to support quality care, service excellence, and productivity goals.
- Ensure consistent implementation of individualized care plans and adherence to hospice standards across interdisciplinary teams.
- Facilitate and lead Interdisciplinary Team (IDT) meetings.
- Promote accountability,timelycare and communication,high-quality documentation,and professional practice.
- Conduct ride-alongs, shadowing, coaching sessions, and chart reviews to support professional growth and clinical excellence.
- Supportstaffthroughcoaching,feedback,andongoingperformance improvement initiatives.
- Ensure clinical services are available 24 hours per day, 7 days per week, including participation in a manager on-call rotation and providing clinical support as licensure permits.
- Collaborate with physicians, the Medical Director,Director ofPatient
Care,and interdisciplinary team members tocoordinatepatientcareandsupportpatientandfamilygoals. - Build andmaintainstrong relationships with healthcare facilities, referral partners, and community stakeholders.
- Serve as an ambassador for Emmanuel Hospice, promoting our mission, services, and philosophy of care.
