Description:

The RN Clinician role on the inpatient Kidney Transplant team is located at Mercy Health Saint Mary's in Grand Rapids, Michigan. This is a full-time role. This position will cover waitlist management (phone triage, coordinating surgeries, re-evaluations, etc.). Willingness to cross-train to all areas of the Transplant team. POSITION SUMMARY

Oversees and coordinates care for Kidney Transplant area for an assigned caseload. Facilitates appropriate care, assists patients and families in finding and negotiating that care. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and cost effective care.

POSITION QUALIFICATIONS
  • Bachelors of Science in Nursing.
  • Masters of Science in Nursing preferred.
  • Current State of Michigan Registered Nursing License, in good standing.
  • Specialty professional Certification preferred, yet required to obtain within 2 years.
  • Minimum of 3 years in nursing experience.
ADDENDUM: KIDNEY TRANSPLANT CENTER

Establishes and promotes collaborative relationships with members of the transplant multidisciplinary team in order to develop plans that optimize the patient's success throughout the transplant process. Responsible for coordinating all clinical aspects of the patients; including patient care, education, quality assurance, psychosocial and financial assessments. Coordinates evaluation process for pre-transplant patients as well as potential donors. Advocates on behalf of patients and families within the transplant multidisciplinary team and the community.

Assesses post-transplant organ function and general health status and collaboratively determines need for additional assessment or intervention. Facilitates the scheduling of tests and procedures for diagnostic purposes. Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team. Completes necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing, status changes, post-transplant follow-up.

Participates in team conferences, rounds, or meetings in relation to care of the transplant patient or program. Be a part of a collaborative and respected team.
