Job Summary

Up to $10,000 in Sign on Bonus Incentives for Licensed RN's who will be new to the RN role for Corewell Health and meet eligibility requirements! This position will service the Greater Grand Rapids area of Home Care Job Summary Visiting Nursing Association a great place to start your home health career. We offer flexible work schedules including working every 4th weekend, working every 3rd holiday and on call 1-2 days in a 6 week rotation.

Working in home health enables you to treat the patient in the comfort of their home where patients are able to thrive and own their own health care. • Communicates effectively with staff, residents and families.
• Assesses, develops, and maintains an individualized patient plan of care with the patient
• Performs Medication Administration Pass according to regulatory guidelines.
• Informs MDS coordinator of significant resident/patient changes needed for completion of the MDS.
• Communicates with other members of the health care team in order to ensure continuity of care and coordination of services.
• Acts as a patient/family advocate.
• Evaluates patient’s and significant other’s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
• Observes and reports the patient’s condition, needs and outcomes of intervention to the physician and other team members.
• Identify problems and emergency situations, and initiate immediate “life saving” measures in the absence of a physician.
• Teaches and supervises the care provided by CNA, LPN or other nursing service personnel according to regulatory and agency guidelines.
• Implements the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating the patient’s progress toward established goals recommending revisions to the plan as necessary, and planning for termination of service or discharge.
• Assumes a leadership role and delegates appropriately to support staff to facilitate safe, quality patient care in a cost-effective manner.
• Participates in ongoing process improvement activities through the application of clinical paths, QA processes and other related activities.
• Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency.

• Required Associate's Degree Nursing 2 years of relevant experience OB-postpartum and breastfeeding support, specific to the Going Home RN for Healthier Communities
• LIC-Registered Nurse (RN) -  State of Michigan Upon Hire
• LIC-Driver's License
  - State of Michigan Upon Hire
• CRT-Basic Life Support (BLS) - AHA American Heart Association VNA and Home Care Business lines 90 Days Or
• CRT-Basic Life Support (BLS) - ARC American Red Cross VNA and Home Care Business lines 90 Days
• Preferred Bachelor's Degree Nursing 1 year of relevant experience of acute care, emergency, or rehab facility experience for all of the Home Care business lines
• CRT-Board Cert Lactation Consultant (BC-IBCLC) - IBCLCE International Board of Lactation Consultant Examiners Going Home RN for Healthier Communities Upon Hire
