Role Description

This full-time, on-site Nurse Practitioner role is based in Grand Rapids, MI, and involves delivering direct patient care in a family medicine setting. Day-to-day responsibilities include performing patient assessments, obtaining medical histories, conducting physical exams, ordering and interpreting diagnostic tests, and developing treatment plans in collaboration with physicians and other clinicians. The Nurse Practitioner will manage acute and chronic conditions, prescribe medications as appropriate, and provide preventive care, health counseling, and patient education.

The role also includes documenting encounters in the electronic health record, coordinating referrals, participating in quality improvement initiatives, and working closely with the care team to ensure safe, efficient, and patient-centered care.

Qualifications
  • Current Nurse Practitioner licensure and certification in the state of Michigan, with eligibility to practice in a family medicine or primary care setting.
  • Proficiency in patient assessment, diagnosis, and treatment planning for a wide range of acute and chronic conditions.
  • Strong skills in patient education, counseling, and preventive care, with the ability to communicate clearly and compassionately.
  • Experience using electronic health records and maintaining accurate, timely clinical documentation.
  • Ability to collaborate effectively with physicians, nurses, medical assistants, and administrative staff in a team-based care environment.
  • Demonstrated commitment to evidence-based practice, quality improvement, and patient safety.
  • Master’s degree in Nursing or related field from an accredited program; previous experience in family medicine, internal medicine, or primary care is preferred.
  • Strong organizational and time-management skills, with the ability to work efficiently in a busy outpatient practice.
