Works as part of a primary care team including registered nurses, administrative support staff, registered dietitians, behavioral health staff, and clinical students from a variety of healthcare disciplines. Responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health center. Maintains a collaborative practice agreement with supervising physician. Exercises independent judgment in the assessment, diagnosis and initiation of selected protocols/procedures.
Provides educational support for patients/significant others, health team members and the community. Mentors or precepts students and volunteers assisting in any of these activities.
- Holds a master's or doctoral degree in nursing from an accredited nurse practitioner program
- Licensed as a Registered Nurse in the State of Michigan
- Certified and licensed as a Nurse Practitioner in the State of Michigan
- Obtains comprehensive histories and physical examinations that reflect the acuity and presenting complaint of the patient.
- Documents/dictates progress notes that appropriately capture the content of the obtained history and physical.
- Functions in the role of an advanced practice provider to coordinate and deliver comprehensive medical/nursing care including following treatment protocols and pathways, writing orders/medications to support patient’s plan of care, ordering and interpreting laboratory and radiographic studies, and seeking consultation from supervising physician as needed to achieve optimal patient outcomes.
- Collaborates with support staff and integrated team members from other disciplines to optimize patient care outcomes effectively. Acts as a resource to nursing, support staff and students as appropriate.
- Communicates in a timely and effective manner with supervising physician as needed for continuity of care and optimal patient outcomes.
- Serves as a patient advocate and provides patient education and/or referrals for optimal outcomes and continuity of care. Communicates effectively with patients and family members, demonstrating sensitivity to their individual needs and concerns.
- Performs other tasks, procedures, or duties as assigned within the scope of practice.
