Candidate Type:Traveler or local candida ted accepted
Requirements:1 year of Medicare Certified Home Health experience. 1 year of HCHB EMR and OASIS experience
Responsibilities:
- Provide comprehensive nursing care to home health patients.
- Assess patient needs and develop individualized care plans.
- Monitor patient progress and update healthcare team accordingly.
- Educate patients and their families on health management and disease prevention.
- Document patient care activities accurately and timely.
