Licensed Practical Nurse (LPN) - Home Visits
Atrio Home Care- a partnership between Holland Home, Brio and Rest Haven- is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care throughout the Grand Rapids or Holland territory. This role focuses on delivering high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of downtown Grand Rapids or downtown Holland/Lakeshore.
Schedule:Full-time or part-time opportunities available
Hours:8:00 AM - 5:00 PM, with flexibility in scheduling your visits
Schedule:Mon-Fri;
No weekends or On-Call
35-mile radius from downtown Grand Rapids
Key Responsibilities- Deliver skilled nursing care under RN supervision, including wound care, dressing changes, catheter care, and medication/injection administration
- Follow and support individualized Plans of Care in collaboration with physicians and the care team
- Provide specialized nursing services such as preventive care (skin care, pressure relief) and chronic disease management
- Educate clients and caregivers on disease processes, self-care, and health promotion to encourage independence
- Maintain accurate, timely documentation using the Matrix EMR system (iPads provided)
- Current Michigan LPN license
- Valid driver's license, reliable transportation, and proof of auto insurance
- 1-2 years of acute care or skilled nursing experience (home health preferred)
- Passion for delivering exceptional care to older adults
