* Position Summary:

* Join our compassionate ICU team at Trinity Health Grand Rapids Hospital, where your skills in clinical social work will make a critical impact during some of the most vulnerable moments in our patients' lives. In this fast-paced environment, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your work will help patients navigate crisis situations, overcome barriers, and access needed community resources to ensure safe and effective care transitions.
** What You'll Do:*
* + Provide social work services during the 8:00 AM - 4:30 PM shift in the ICU.
+ Conduct thorough psychosocial assessments for patients and families in crisis.
+ Develop individualized treatment and discharge plans that address emotional, social, and practical needs.
+ Deliver brief counseling and crisis intervention.
+ Coordinate with community agencies and internal resources to support safe care transitions.
+ Serve as a patient advocate and liaison between care teams and families.
+ Document assessments, plans, interventions, and outcomes in a timely and accurate manner.
+ Collaborate with the interdisciplinary care team to support holistic patient care.
** What You'll Bring:*

* Education:
** Master's Degree in Social Work or a related field from an accredited institution.

** Licensure:
** Eligible for licensure as a
** Limited Licensed Master Social Worker (LLMSW)
** or
** Licensed Master Social Worker (LMSW)
** by the Michigan Board of Social Work.

* Experience:
** Minimum of one year of social work experience in a healthcare setting; experience in emergency or acute care preferred.
