ICU Social Worker, Day Shift M-F 8a-4:30p

Join our compassionate ICU team at Trinity Health Grand Rapids Hospital, where your skills in clinical social work will make a critical impact during some of the most vulnerable moments in our patients' lives. In this fast-paced environment, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your work will help patients navigate crisis situations, overcome barriers, and access needed community resources to ensure safe and effective care transitions.

Provide social work services during the 8:00 AM – 4:30 PM shift in the ICU.

Conduct thorough psychosocial assessments for patients and families in crisis.

Develop individualized treatment and discharge plans that address emotional, social, and practical needs.

Deliver brief counseling and crisis intervention.

Coordinate with community agencies and internal resources to support safe care transitions.

Serve as a patient advocate and liaison between care teams and families.

Document assessments, plans, interventions, and outcomes in a timely and accurate manner.

Collaborate with the interdisciplinary care team to support holistic patient care.

Education:

Master's Degree in Social Work or a related field from an accredited institution.

Licensure:
Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.

Experience:

Minimum of one year of social work experience in a healthcare setting; experience in emergency or acute care preferred.

Exceptional crisis intervention and psychosocial assessment skills.

Strong emotional resilience and sound decision-making under pressure.

Compassionate communication and active listening.

Collaborative team player who thrives in a dynamic, high-acuity environment.

Familiarity with local resources and referral processes for at-risk populations.

Accurate, timely clinical documentation and case tracking.
