Social Worker- Palliative Care Manager

Emmanuel Hospice in Grand Rapids, MI seeks a Social Worker - Palliative Care Manager to provide holistic, faith-informed support to patients and families facing serious illness. In this role, you will lead and coordinate palliative social work services, conduct biopsychosocial assessments, and develop person-centered care plans. You'll collaborate closely with nurses, chaplains, physicians, and volunteers to address emotional, social, spiritual, and practical needs.

Responsibilities include care navigation, crisis intervention, family meetings, community resource referrals, documentation, and participation in quality and program development initiatives.
