The RoleAs a Housing Stabilization Case Manager, your work helps neighbors who’ve experienced chronic homelessness keep their housing and move toward healthier, steadier lives. You’ll provide individualized, trauma-informed case management that strengthens stability and reduces returns to homelessness.
Key responsibilities:
- Build client-centered plans focused on housing retention, wellness, and financial stability
- Coordinate resources and services; connect clients to community supports
- Use motivational interviewing to coach progress toward goals
- Maintain timely, accurate documentation using databases and common office tools
Required qualifications/skills:
- Bachelor’s in social work/human services or relevant human services experience (preferred)
- Case management experience (preferred); lived experience with homelessness (preferred)
- Valid driver’s license and willingness to transport clients
- Strong written/verbal communication, problem-solving, and adaptability
Your morning may start with a check-in by phone or video, then shift to in-home visits where you listen closely and help clients stay on track with the routines that protect their housing. Some days include arranging or providing transportation to appointments, then returning to document notes and update plans. You’ll also spend time coaching practical skills—like meal prep or basic housekeeping—while reinforcing progress and celebrating small wins.
Throughout the week, you’ll coordinate with your program manager and fellow HSCMs to stay aligned and responsive as client needs change.
